Irritable bowel syndrome (IBS) is a clinical diagnosis grounded in the Rome IV Criteria—recurrent abdominal pain (≥1 day/week in the last 3 months) associated with defecation or change in stool frequency/form. Avoid reflexive over-testing; start with a focused history, medication review, and limited labs (CBC, CRP, celiac screen) unless red flags (weight loss, anemia, GI bleeding) are present.
Treatment highlights APPs:
- Subtype matters: IBS-D vs. IBS-C guides therapy.
- Diet first: Consider a low-FODMAP trial and fiber optimization (psyllium preferred over insoluble fiber).
- Targeted pharmacology:
- IBS-C: osmotic laxatives, secretagogues
- IBS-D: antidiarrheals, bile acid binders, or gut-directed antibiotics when appropriate
- Address the gut–brain axis: Low-dose TCAs or SSRIs and behavioral therapies (CBT, gut-directed hypnotherapy) can improve global symptoms.
- Patient education is key: Set expectations—IBS is chronic, but manageable.
